| NHS Prescriptions |

| If you are exempt from
prescription charge please tick the relevant
exemption box and send us a photocopy of your
exemption evidence (if applicable). You will
need to send this proof of exemption only once. |

| The current charge for NHS
prescriptions is £6.85 per item. |
| |
| If paying for item(s) on an NHS
prescription please use the ‘NHS Prescription
Charge’ product from the shop area. |

| If your prescription contains more than one item you may need to increase the product quantity to the number of item the prescription holds. |

| Private Prescriptions |

| Send us your private
prescription using our free post address, Cohens
Chemist On-Line Pharmacy will then calculate the
cost and phone or e-mail you with the details
and how to pay. |
